Complete Survey
Answer questions about health, family history, and lifestyle
It may take 1-3 hours to complete all 4 survey modules.
Submit Saliva Sample
To be securely stored for future research
Upon completion of the 2nd survey module, participants will receive their individual DNA kit. The saliva sample will be processed for genetic information, which will be used to create a large biorepository for future research about healthy aging.
You are welcomed to have a trusted friend or family member help you complete all study activities.
